J.D. is torn between participating in Dr. Kelso's case study contest and gaining the respect of Dr. Cox. After experiencing awkward silence when left alone together, Elliot and Carla realize that they have nothing in common except J.D. and Turk. Elliot attempts to address the issue by finding ordinary things they have in common, while Carla doesn't even think it is an issue. When Elliot and Carla prep Mrs. Kellerman, a sixty year old woman, for breast implants, they begin thinking about what they would change about themselves and realize that they actually do have a lot in common.
