I wish you could give half stars on this review system, because I'd prefer to give Shortbus 3.5, but I can't. Shortbus is a watchably confrontational dramedy about relationships, love and sex, sex, sex. I think to just focus on the sex scenes (as graphic, confronting and intense as some of them are) is to miss a lot of the quite fun dialogue, characterisations and plot. Okay, the acting is pretty woeful, but there is something affectingly genuine about it all. Yes, the ending is anticlimactic (haha), but all in all it's not too bad, but not brilliant either. I must also confess that Hedwig and the Angry Inch is one of my all-time favourite films, though, and I do struggle to say a bad word against John Cameron Mitchell because of how much I love and respect his ideas and his previous film. So, in summary, if you are worried about whether or not you can handle the graphic sex scenes, then just see if you can pass the first five minutes without feeling overwhelmed - if so, you will be able to handle the rest of the film. On the other hand, if you are getting this because you want some pseudo-porn, you will be disappointed as the sex is frank and real and not particularly erotic.
